Is letrozole FDA-approved for fertility?
The FDA label covers breast cancer in postmenopausal women and nothing else, so the answer is no. Using it to induce ovulation is off-label, which the 2023 international guideline states outright: the use of ovulation induction agents, including letrozole, metformin and clomiphene citrate, is off-label in many countries. Off-label prescribing is legal and routine in fertility care, but you are entitled to hear it named before your first cycle.
Does letrozole increase the risk of birth defects?
The evidence built since the 2005 scare says no. A study of 911 newborns found major malformation rates of 1.2% with letrozole against 3.0% with clomiphene, and cardiac anomalies in 0.2% against 1.8%, P=0.02. Will letrozole help if you do not have PCOS?
