How Letrozole Induces Ovulation

Letrozole is an aromatase inhibitor. Its FDA label covers adjuvant and extended adjuvant treatment of postmenopausal women with hormone receptor positive early breast cancer, and first and second-line treatment of advanced breast cancer. Nothing on that label mentions ovulation, conception or infertility. Every prescription written for fertility in the United States is off-label, and the 2023 international guideline says it plainly: the use of ovulation induction agents, including letrozole, metformin and clomiphene citrate, is off-label in many countries. Off-label is the ordinary status of this whole drug class rather than a red flag, but your prescriber should say it out loud before you start.
The mechanism is a deliberate, temporary hormone crash. Short-term administration during the early follicular phase suppresses ovarian and peripheral estrogen synthesis by approximately 97 to 99 percent, producing a transient hypo-estrogenic milieu. How Effective Is Letrozole for Fertility?

The trial that moved practice is PPCOS II, published in 2014. It randomized 750 women with PCOS to letrozole or clomiphene for up to five cycles, with live birth as the primary outcome rather than a surrogate. Women who received letrozole had more cumulative live births than those who received clomiphene, 103 of 374 (27.5%) versus 72 of 376 (19.1%), P=0.007, rate ratio 1.44, 95% confidence interval 1.10 to 1.87. Read that as roughly one extra live birth for every twelve treated, which is a meaningful edge and not a transformation.
Ovulation moved the same way. The cumulative ovulation rate was higher with letrozole than with clomiphene, 834 of 1352 treatment cycles (61.7%) versus 688 of 1425 cycles (48.3%), P<0.001. Notice what did not move. There were no significant between-group differences in pregnancy loss, 49 of 154 pregnancies in the letrozole group (31.8%) and 30 of 103 in the clomiphene group (29.1%). Close to a third of pregnancies in both arms ended in loss. Letrozole improves your odds of ovulating and conceiving in a given cycle. It does nothing measurable to protect a pregnancy once it has started, and any service implying otherwise is selling you something the trial never showed.

The Thin Lining Argument, Examined

Clomiphene is a selective estrogen receptor modulator, so it occupies estrogen receptors well beyond the pituitary, and it lingers. Its own label reports that zuclomiphene has a longer half-life than enclomiphene, with detectable levels persisting for longer than a month, while the prolonged anti-estrogenic activity of clomiphene may adversely affect estrogen-sensitive tissues such as the endometrium and cervical mucus. Who Letrozole Works For

Letrozole fixes exactly one problem: you are not releasing an egg reliably. The guideline recommendation is narrow and the qualifier at the end is the part that gets dropped in marketing copy. Letrozole should be the first-line pharmacological treatment for ovulation induction in infertile anovulatory women with PCOS, with no other infertility factors. If ovulation is not your bottleneck, a drug that induces ovulation changes nothing about your odds, and it costs you cycles finding that out.
The trial's entry criteria draw the boundary better than any checklist. PPCOS II enrolled participants who were 18 to 40 years of age, had at least one patent fallopian tube and a normal uterine cavity, and had a male partner with a sperm concentration of at least 14 million per milliliter. Tubal blockage, significant male factor and diminished ovarian reserve were screened out before randomization, so the trial says nothing about them.
In those situations letrozole is not a slower route to the same destination. It is the wrong lever, and a tubal patency test and a semen analysis belong before cycle three rather than after cycle six.

Against clomiphene the guideline is direct: letrozole should be used rather than clomiphene citrate in women with PCOS with anovulatory infertility and no other infertility factors to improve ovulation, clinical pregnancy and live birth rates. On twins the two drugs sit closer than reputation suggests. Cochrane reported multiple pregnancy rates of 2.2% with SERMs versus 1.6% with letrozole, odds ratio 0.74, 95% CI 0.42 to 1.32, high-certainty evidence, and PPCOS II found no significant difference in twin pregnancy, 3.4% and 7.4%.
Injectable gonadotropins are the sharper contrast. In the AMIGOS trial of unexplained infertility, live birth occurred in 32.2% of gonadotropin cycles, 23.3% of clomiphene cycles and 18.7% of letrozole cycles, while the multiple gestation rate was 32% with gonadotropin alone against 13% with letrozole, P=0.006. Participants received letrozole 2.5 mg daily beginning on cycle day 3 for 5 days, for up to five menstrual cycles, after either a spontaneous period or a progestin-induced withdrawal bleed. Letrozole Side Effects and the Pregnancy Contraindication
