How Sildenafil and Tadalafil Work

Sildenafil and tadalafil are the same class of drug doing the same job. Both inhibit phosphodiesterase type 5, the enzyme that breaks down cyclic GMP in the smooth muscle lining penile arteries. Block it, cGMP lingers, the muscle relaxes and blood flows in. The sildenafil label is explicit that the drug has no effect in the absence of sexual stimulation, and tadalafil works the same way. Neither creates desire. Both amplify a signal your body has already started.
Where they split is how long the molecule sticks around. Sildenafil and its active metabolite have terminal half lives of about 4 hours. Tadalafil has a mean terminal half-life of 17.5 hours, roughly four times longer. Since the AUA describes the mechanism of every approved PDE5 inhibitor as similar, picking between them is rarely a question of which is stronger. It is a question of when you want coverage, what you eat, which side effects you would rather avoid, and whether prostate symptoms are also in play.

How Long Sildenafil and Tadalafil Last

The sildenafil label puts the dose roughly an hour before sexual activity and allows a window of 30 minutes to 4 hours. Maximum plasma concentration arrives in 30 to 120 minutes, median 60. That is a scheduled drug in the ordinary sense: you decide, you dose, you wait. Miss the window and the drug has not vanished, but you are outside the conditions the label was written for.
Tadalafil is different in kind. Its label states the as-needed dose improves erectile function compared to placebo up to 36 hours following dosing, with peak concentration reached between 30 minutes and 6 hours, median 2 hours. The 36-hour phrase in advertising is shorthand for that property rather than an invented slogan. The AUA tells prescribers to explain that the medications differ in onset of action, duration of action, and whether food intake limits efficacy. Slower, less predictable onset buys a window that spans a weekend.

Is Sildenafil or Tadalafil More Effective?

Anyone telling you one of these is definitively better has picked a meta-analysis and ignored the other. The trade-off network meta-analysis by Chen and colleagues, pooling 82 trials and 47,626 participants for efficacy, found that among starting doses sildenafil 50 mg had the greatest efficacy but also the highest rate of overall adverse events, while tadalafil 10 mg had intermediate efficacy and the lowest overall adverse event rate of any agent studied. Yuan and colleagues, pooling 118 trials and 31,195 men, reached a different ranking. Across efficacy outcomes the absolute effects and rank tests put tadalafil first, followed by vardenafil, with no major difference in the safety profile between agents. Both analyses are competent. They disagree because they weighted efficacy and tolerability differently, and because the underlying literature is thin on trials that compared these two drugs head to head at matched doses.
The AUA lands where the evidence actually sits. The PDE5 inhibitors appear to have similar efficacy in the general ED population, and the guideline states plainly that the literature lacks trials comparing medications to each other. That is not a dodge. With close to a quarter of a million men studied on this drug class, a large efficacy gap would have surfaced. The differences that survive scrutiny are practical ones, and they fill the rest of this page.

Does Food Affect Sildenafil or Tadalafil?

This is the most useful paragraph on the page. The sildenafil label records that with a high fat meal the rate of absorption is reduced, with a mean delay in Tmax of 60 minutes and a mean reduction in Cmax of 29%. A heavy dinner, then a pill, then disappointment an hour later is not a drug failure.
It is a food effect, and it is printed on the label. The label's own consumer wording says the drug may take longer to start working after a cheeseburger and fries.
Tadalafil has no equivalent problem. Its label is blunt that the rate and extent of absorption of tadalafil are not influenced by food, and the AUA comparison table records the same split across the class. The scale of this matters more than either fact alone: studies of men reporting non-response found that incorrect use, including taking the medication with a large meal or without sexual stimulation, accounts for 56% to 81% of treatment failures, and after re-education 23.6% to 58.5% went on to treatment success.

Daily Tadalafil vs As-Needed Dosing

The tadalafil label carries a once-daily option that sildenafil has no counterpart to. For erectile dysfunction the recommended starting dose for once daily use is 2.5 mg, taken at approximately the same time every day, without regard to timing of sexual activity, and may be increased to 5 mg. Sildenafil is licensed as-needed only, with a maximum dosing frequency of once per day and no maintenance regimen anywhere in its label.
What daily dosing buys is not extra potency. It is the removal of a decision. Nothing has to be timed, nothing has to be taken in front of anyone, and there is no window to fall outside of. That suits a regular relationship, or a reader whose anxiety attaches to the pill itself. There is trial support for switching rather than escalating: the AUA cites work in which 623 men with suboptimal results on maximum-dose on-demand therapy were offered daily tadalafil at 2.5 mg or 5 mg against placebo for 12 weeks. Whether that route fits you is a prescribing decision.

Sildenafil vs Tadalafil Side Effects

The side effect profiles diverge in a way the chemistry predicts. In fixed-dose trials the sildenafil label reports abnormal vision, described as a color tinge, light sensitivity or blurring, at 1% on 25 mg, 2% on 50 mg and 11% on 100 mg against 1% on placebo, with flushing at 19% on 50 mg and 18% on 100 mg against 2% on placebo. Both effects were described as mild to moderate and transient.
Tadalafil's characteristic complaints are musculoskeletal. Its pooled table shows back pain at 6% and myalgia at 3% on the 20 mg dose against 3% and 1% on placebo, with flushing at only 3% against 1%. Visual disturbance does not appear in that table at all. Neither drug escapes headache: sildenafil ran 21% on 50 mg and tadalafil 15% on 20 mg, against placebo rates of 7% and 5% respectively.
The mechanism behind the split is off-target selectivity. Sildenafil is only about 10-fold as potent for PDE5 compared to PDE6, the enzyme that handles phototransduction in the retina, which is why color discrimination shifts at higher doses. Tadalafil is 14-fold more potent for PDE5 than for PDE11A1, an enzyme found in skeletal muscle, prostate and testes; the label notes the clinical consequence is not established, though the back pain signal tracks it. That is a real basis for preferring one over the other.

Tadalafil for BPH and Prostate Symptoms

This is the clearest non-tie on the page. Tadalafil is indicated for erectile dysfunction, for the signs and symptoms of benign prostatic hyperplasia, and for both together, at 5 mg once daily. Sildenafil's label carries only the erectile dysfunction indication. If you are up twice a night and also struggling with erections, one of these drugs is labeled for both problems and the other is labeled for one.
The evidence behind that indication is meaningful without being miraculous. Gacci and colleagues pooled seven placebo-controlled trials covering 3,214 men and found PDE5 inhibitors improved the International Index of Erectile Function by 5.5 points and the International Prostate Symptom Score by 2.8 points, while maximum flow rate did not move. Added to an alpha-blocker across five trials with 216 men, the combination improved IIEF by 3.6, IPSS by 1.8 and peak flow by 1.5. Symptoms improve; the measured plumbing barely does.

Nitrates, Alpha-Blockers and Other Interactions

One rule here has no flexibility in it. The tadalafil label states that administration to anyone using any form of organic nitrate is contraindicated, and the sildenafil label carries the same bar. That covers nitroglycerin tablets and sprays, isosorbide, and recreational nitrites, whether taken regularly or occasionally. Both labels also contraindicate guanylate cyclase stimulators such as riociguat. No dose is small enough to make either combination acceptable.
Alpha-blockers, prescribed for prostate symptoms and for blood pressure, are a caution rather than a bar. Both classes are vasodilators, so the tadalafil label warns that an additive effect on blood pressure may be anticipated. Sildenafil's label directs that alpha-blocker therapy be stable first and that sildenafil should be initiated at 25 mg in that case. Sildenafil vs Tadalafil Cost

Both drugs are generic in the United States and the price collapse is not subtle. The federal NADAC survey, which reports what retail pharmacies pay to acquire a drug, listed generic sildenafil 100 mg at $0.128 per tablet and generic tadalafil 20 mg at $0.158 per tablet in the file published on 2 September 2026. Tadalafil 20 mg carries two live listings there, $0.15815 across 31 of its 40 records and $0.14002 across the other nine, and the figure above is the majority one. Brand-flagged rows on the same file put Viagra 100 mg at $83.63 and Cialis 20 mg at $53.84 per tablet.
Generic tadalafil 5 mg was $0.102 per tablet on that survey, close to $3 for a thirty-day supply at acquisition cost, and generic sildenafil 50 mg was $0.106. Be clear what these figures are: pharmacy acquisition cost. They are neither your retail price nor a telehealth subscription price, which routinely runs many multiples of it. Ask any online service for the per-tablet price and whether the quote assumes a subscription, and check the pharmacy behind it against the FDA's BeSafeRx guidance. A site selling either drug with no prescription is not operating legally.

How to Choose Between Sildenafil and Tadalafil

Selection criteria that track the evidence, rather than a winner. If your sex life is scheduled and you can dose on a light stomach, sildenafil's four-hour window costs you nothing, and Chen's analysis suggests 50 mg is the strongest of the starting doses. If it is not scheduled, or dinner is usually involved, tadalafil's food-independence and 36-hour window remove two ways for the attempt to fail before it starts.
If prostate symptoms sit alongside the erectile ones, tadalafil's BPH indication settles it. If you take an alpha-blocker, say so, because it changes the starting dose. If color tinting or flushing bothered you on sildenafil, the PDE6 selectivity behind it belongs to that molecule specifically, so a switch is reasonable rather than futile. If back pain bothered you on tadalafil, a lower daily dose is a conversation worth having.
Two preference trials get quoted at you constantly, and both deserve their doses in view. von Keitz reported 132 of 181 evaluable participants, 73%, choosing tadalafil, and Govier reported 126 of 190, 66.3%. Both compared tadalafil at its top 20 mg dose against sildenafil at its 50 mg starting dose, which is not a matched comparison.
